CCRN Test 04: Clinical Priorities

This exam reviews clot-busting drugs, lung taps, and heart emergencies.

Stroke Safety: Never give clot-busting drugs (tPA) to a patient who has had a brain bleed. It can be fatal.
Heart Check: If you see extra heart beats (PVCs), check the patient first. Check their pulse and BP before calling the doctor.
Clean Care: Once a bottle of sterile water is opened, you cannot use it again for a sterile field. The lid is dirty.

Q:1What is a reason to NEVER give clot-busting drugs (tPA) to a stroke patient?
A brain bleed in the past means the risk of a new, deadly bleed is too high for these drugs.
Q:2Which fluid lab result means there is pus (empyema) in the lung space?
Bacteria eat the sugar and make the fluid acidic. This leads to low pH and low glucose.
Q:3Which drug should be given first for a blood pressure of 268/180 from a tumor (pheochromocytoma)?
This tumor floods the body with stress signals. You need a drug that opens blood vessels fast.
Q:4A heart monitor shows extra heart beats every other beat (bigeminy). What do you do first?
Always check the patient first. You need to know if the extra beats are hurting their blood flow.

Test 04 Key Study Points

1. Empyema Pus in the lung space has very low sugar and a low pH. It means a bad infection is present.
2. Breathing Tubes Keep the cuff pressure below 22 mmHg (30 cmH2O). Too much pressure can hurt the windpipe.
3. Blood Transfusions Only use 0.9% Normal Saline with blood. Other fluids can make blood cells pop or clump.
4. Legal Proxy If a patient cannot think clearly, their legal guardian or health proxy must sign for surgery.
