NCLEX-PN Test 03: Essential Clinical Safety

This exam tests your knowledge of Emergency Actions, Infant Development, and Oncology Care.

Evisceration Emergency: If a wound opens and organs show, do not push them back in. Cover them with a sterile dressing soaked in saline right away.
Medication Safety: Carbamazepine can cause the body to stop making white blood cells. A very low WBC count is a life-threatening sign.
Cardiac Devices: Patients with an ICD should use cell phones on the ear opposite to the device to prevent electrical interference.

Q:1A baby weighs 7 pounds when born. What is the lowest weight the nurse expects this baby to be at 1 year old?
A baby should weigh three times their birth weight by age one. Since 7 times 3 is 21, the baby should weigh at least 21 pounds.
Q:2A client has a bone tumor called Ewing's sarcoma. Which symptom should the nurse expect to find?
This type of cancer grows in the bone marrow. Persistent pain in the affected bone is the most common sign of this disease.
Q:3A client with epilepsy takes carbamazepine. Which lab result shows a dangerous problem with the blood?
A WBC count of 2,000 is very low. This medicine can stop the body from making enough cells that fight infection, which is very dangerous.
Q:4A 6-month-old baby might have a blocked intestine (intussusception). Which question is the least helpful for the nurse to ask the parents?
This is an emergency where the intestine slides into itself. Questions about pain, vomiting, and swelling are more important than what the baby usually eats.

Test 03 Study Summary

1. Infant Growth A baby should triple their birth weight by their first birthday. Early weight loss of up to 10% is normal.
2. Post-Op Care Suctioning can slow down the heart rate by hitting the vagus nerve. Always monitor for a slow pulse (bradycardia).
3. GI Health Fiber is good for wellness, but the bowel needs rest during a flare-up. Switch to low-fiber foods like white rice when the colon is inflamed.
